Research and intervention on nutritionally assisted treatment of malignant lymphoma

Department of Haematology, Shanghai Changhai Hospital Dong Qunsheng

【Abstract】Malignant lymphoma is a malignant tumour of the high-metabolism and high-consumption blood system. Abnormal tumour proliferation causes the body's systemi inflammatory reaction and metabolic reprogramming, and the damage of radiotherapy to the digestive barrier and immune system. The incidence of malnutrition, muscle attenuation and immunosuppression in patients is significantly higher than that of ordinary solid tumours, directly

Reduce the tolerance of antitumour treatment, prolong the hospitalisation cycle, and affect the long-term prognosis. Scientific nutrition-assisted treatment is an important part of the comprehensiveand standardised diagnosis and  treatment of lymphoma. It can achieve efficacy and toxicity reductionby correcting metabolic disorders, regulating the  tumour microenvironment, and repairing the body's immune barrier. In combination with the clinical practice of haematological tumour diagnosis and treatment, this paper deeply explores the pathological mechanism, application value and existingshortcomings of the whole-process nutritional intervention of malignant lymphoma, and puts forward the idea of individualised, layered and periodic dietary nutrition intervention. Research confirms that

accurate nutritional support throughout the whole process can effectively improve the metabolicstatus of patients and  reduce the incidence of complications, which is the key path to realise"nutrition-enabled disease prevention and control".

【Discussion】In the pathogenesis of malignant lymphoma, there is a significant phenomenon of tumour metabolism reprogramming. The body changes from normal energy metabolism to abnormal metabolic mode based on anaerobic glycolysis, which continues to consume skeletal muscle and visceral protein, and induces chronic consumptive malnutrition and low-grade inflammatoryhomeostasis. Although chemotherapy, targeted and immunotherapy can  inhibit tumour proliferation, they will further damage the gastrointestinal mucosal barrier, inhibit bone marrow haematopoietic function, cause anorexia, absorption disorder, intestinal flora disorder, and form a double metabolic

imbalance of "tumour consumption + iatrogenic injury", which seriously affects the continuity oftreatment. Precise nutrition-assisted treatment can target the pathological metabolic mechanism of lymphoma patients. By optimising the ratio of high-quality protein, branched-chain amino acids, unsaturated fattyacids and antioxidant nutrients, it can  effectively inhibit the body's excessive inflammatory reaction, delay muscle decay, repair intestinal barrier function, and improve the body's tolerance to radiationchemotherapy and immunotherapy. At the same time, scientific dietary  intervention can improve the activity of the body's immune cells, reshape the anti-tumour immune microenvironment, reduce the incidence of adverse reactions related to immunotherapy, and achieve the synergy of nutritional

support and clinical treatment. At present, there are still obvious shortcomings in clinical practice: first, lymphoma nutrition intervention is mostly a general model, and there is a lack of layered intervention plan based on tumour type, treatment stage, body mass index and inflammatory indicators; second, clinical anti-tumour treatment, light metabolic nutrition management, and the whole-process nutritionscreening and dynamic evaluation system are missing; third, the  patient There are widespread dietary misunderstandings such as excessive taboo and blind nourishment, which aggravate metabolic disorders and restrict the improvement of clinical efficacy.

【Summary】The disease management of malignant lymphoma is no longer limited to a single drugtreatment, and  metabolic regulation and nutritional repair have become the core links of the standardised management of tumours throughout the whole process. Relying on scientific diet to empower disease prevention and control, the implementation of phased, individualised and dynamic nutritional auxiliary intervention can effectively correct tumour metabolism disorders, reducetreatment toxic side damage, stabilise the body's immune function, and improve the long- term quality of life of patients. Under the concept of big health and precise nutrition, clinical practice should build

a closed-loop nutrition management system of "screening-assessment-intervention-follow-up", promote the deep integration of tumour treatment and nutritional intervention, and help the whole process of disease prevention and control of malignant lymphoma refined and standardised with scientific diet.
